Indonesian Blood Center is one of the organizations responsible for the provision and management of blood. Blood is one product that is perishable due to the short product life, therefore it needs good management in optimal blood supply arrangement. The difficulty of reaching the optimal balance point of supply by the number of existing demand is now a major risk that often occurs in the supply chain. This study aims to design a mitigation action strategy or risk design treatment to risk agents in the blood supply chain so as to minimize the level of risk that affects the supply of blood. The approach taken to achieve these objectives is to observe the risk agents that occur in each supply chain. The method used in this research using House of Risk approach. Application of the method will result in an Effectiveness to Difficulty Ratio value on Preventive Action. Preventive Action is designed and customized based on the risk agents that occur. The results of this study indicate that there are 11 risk agents that are prioritized in the treatment with 7 mitigation actions that can be used, the expectation can have an impact on the suppression of risk agents in the flow of blood supply chain.
