PT. XYZ is one of the manufacturing companies engaged in manufacturing oil and gas, which in the production
process for the manufacture of pumps is inseparable from all the risks that exist. Implementing Occupational
Health, Safety and Environment (HSE) in the production process did not run as smoothly as desired, causing
several cases of work accidents, such as being exposed to material fragments caused by the production machine
process. By using HIRADC and FMEA analysis methods, several causes of work accidents were found. The
solution that can be delivered is to conduct a briefing regarding the importance of Occupational Health and Safety,
put supervisors in every part of the production process, and provide rewards and punishments for employees who
commit or violate Occupational Health, Safety and Environmental regulations. After identifying and analyzing
with HIRADC, Why to why analysis, and FMEA, several solutions were found, such as conducting briefings
regarding the importance of Occupational Health and Safety and placing supervisors in every part of the
production process and providing rewards and punishments for employees who commit or violate regulations.
